• Bear Lake Community Health Center - Garden City Clinic is located at 115 North 100 West, Garden City, UT 84322. They are open Monday through Friday from 8 AM to 5 PM, and closed on weekends. Their phone number is (435) 946-3660.

Both clinics provide a variety of dental services, including:

• Exams and cleanings
• Fillings
• Extractions
• Root canals
• Dentures
• Partials
• Veneers
• Crowns

The clinics also offer fluoride treatments, sealants, and oral cancer screenings.
